The following changes are necessary for the bride to make after marriage:
Check in the space provided as you complete them.

_____ 3. Change name on car insurance.
_____ 4. Change name on health insurance.
_____ 5. Change name on life insurance.
_____ 6. Change name on voter rolls.
_____ 7. Change name on bank account(s).
_____ 8. Change name on IRA’S/401K’S.
_____ 9. Change name on stock certificates.
_____10. Change name on pension records.
_____11. Change name on credit cards.
_____12. Change name on wills.
_____13. Change name with employer(s).
_____14. Change name on all professional licenses.
_____15. Change name on passport.
_____16. Change name on social security records.
You will need Certified copies of your Marriage License to change the above. Listed below are some instructions that might help you obtain certified copies:
1. Contact the Registrar’s office in the community where the marriage was performed. (This is not necessarily the place where you applied for your license.) Ask if the office has received the license from the officient, and what the fee is for certified copies.
2. Be prepared to present either a photo ID (i.e. picture driver’s license or passport), or
two forms of identification with address. Person requesting copies must complete an affidavit.
Certified copies may only be issued to the following requestors:
Bride or groom – Only the bride or groom may obtain full copies of marriage licenses.
Those listed below may obtain short version (typed copy) with limited information:
Children of the Subject
Legal Guardian (must show proof)
Legal Representative (must show proof)
Court Order
